Do I have to use my official legal name when filling out the form for the badge?

Cuz I use a americanized name but I'm not sure if you're going to require ID's or something which won't match.

Maybe this is a weird question but I'm always worrying about these things...

For the form I would think that using the name on you ID would be best practice. Placing your americanized name in quotation marks on the form for clarity would be even better. When staff are handling your form and ID is when you want them to see you are who you claim to be quickly and easily.
